Effective Dosage of Carbamazepine


Abstract
The study of Tegretol as an Antiepileptic drug (A.E.D.) was begun in Europe in 1959 but in the U.S. its use was first approved for seizure control some 15 years later in 1974.
In Korea, ;however, the drug came to the auther¢¥s hand in large quantity as professional samples as early as 1966 through the Fujizawa Pharmaceutical of Japan which was on a technological tie-up with CIBA/Geigy.
Through it was introduced to Korea and was available for use this early and there were many patients who indicated its prescriptions, the author was hesitant in trying Tegretol, for the periodical and complicated tests for an early detection of its side effects as recommended in Physician¢¥s Desk Reference were not easy in Korea. Then, as he witnessed a water intoxication in the first case of its use volunteered by a patient, the author became more reluctant to the application of the A.E.D.
It was not before 1976 that the author had started prescribing Tegretol in Korea. His motive was not the gain of a drastic therapeutic effect; the use was limited to such cases only where it was necessarily indicated.
At the time, however, the author was always ill at ease as he used to in the early 1960s when he gave his patients Phenurone or Mesantoin. Whenever he received an unexpected call _from a patient on Tegretol, he found himself a victim of the so-called "telephone anxiety."
The author¢¥s decade-long petitions for the solution of these circumstances were heard at last when E.H. kwon, M.D. was appointed the head of the Seoul National University Hospital. Thanks to his consideration, the appeals resulted in the birth of a tox lab for the measurement of serum level of A.E.D. The author has since been able to provide a better quality care for epileptics with a sense of security.
In "A Text Book of Epilepsy" edited by J. Laidlaw et al it is recommended that 600 to 1, 600mg of Tegretol be administered in 3 doses a day, but the auther has not yet found a case that required as much as 1, 600mg. And there was no difference in Tegretol serum level between the patient who had been given it in 2 doses a day and those given in 3 doses.
